Investigations

We conduct formal investigations for the most serious complaints.

We conduct investigations into individual health practitioners where there may be evidence of professional misconduct or the practitioner poses a serious risk to persons.

The Health Ombudsman may also open an investigation into a health facility or service to determine if there are systemic issues affecting the provision or quality of health services.

Systemic investigations

The Health Ombudsman conducts systemic investigations into health service providers and health service issues, making recommendations to improve patient safety and health service quality in Queensland.

Investigation reports

Investigation reports may be released publicly to raise awareness within the health sector and general public of the identified issues and the recommendations made to improve patient safety, and health service quality and delivery in Queensland, as well as on other matters as determined by the Health Ombudsman.

Snapshot reports

Snapshot reports focus on sharing key issues, outcomes and/or learnings arising from a variety of activities undertaken by the OHO to inform and promote improvements in the safety and quality of health service delivery.


List of open investigations

We are required to publicly declare ongoing investigations open for more than one year.

Compliance

We monitor compliance with any immediate action conditions or restrictions imposed on a health practitioner.

Actions/orders taken against health practitioners

The Health Ombudsman can impose restrictions and bans on individual health practitioners. In most cases this occurs when a health practitioner poses a risk to public health and safety or for reasons of public interest.
