Request information
Administrative information
For some non-sensitive information, such as information about you, you can request the information informally. This process is called administrative access.
Individuals may apply for administrative access to documents that:
- were provided to the Office of the Health Ombudsman (OHO) by the applicant
- were provided to the applicant by the OHO
- are publicly available on the OHO website.
Administrative access requests must be in writing and include the following:
- your full name and date of birth
- the postal address where you want the information sent
- details about the documents you are requesting
- *a certified copy of your proof of identity (e.g. driver’s licence, passport, birth certificate, or statutory declaration from an individual who has known you for at least 1 year).

Requesting information under law
Accessing information under the Right to Information Act 2009 and Information Privacy Act 2009
You can also formally request information from the OHO, or any public sector agency in Queensland, under the Right to Information Act 2009 (the RTI Act) and Information Privacy Act 2009 (the IP Act). These Acts give you the right to access and amend such information, unless there is a good reason for it not to be provided. You can apply for access to your personal information under the IP Act and apply for access to personal and non-personal information under the RTI Act.
To make an application you will need to provide the following:
- a completed application form
- a clear scope of the documents you are seeking from our office
- the time period you would like searched
- relevant case or document reference numbers (if possible)
- *a certified copy of your proof of identity (e.g. driver’s licence, passport, birth certificate, or statutory declaration from an individual who has known you for at least 1 year).
If you are seeking documents on behalf of someone else, you must provide certified evidence of both identities as well as proof of your authorisation to act on that person’s behalf (such as a signed authority or power of attorney document).

To make an application, you will need to download and complete the Queensland Government Right to Information and Information Privacy application form.
This form needs to be returned to the Office of the Health Ombudsman directly by email or post.
Please note:
- You will need to first download the form and save it to your computer.
- After saving the form, you can type information directly into the form and save it at any time.
- There may be a fee associated with your request. You will be contacted to discuss this before we process your request.
